Can't find the form you need? Some forms are not available on the Student Records Web site. Check below for info on forms currently NOT available online. If you need a copy of one of the forms below, please come to the Student Records Office.

 

Request to Participate in the Commencement Ceremony Prior to Completing Degree Requirements (Students)
For students who wish to march with their class at commencement while lacking minimal credit toward their degree requirements. It is not for undergraduate students who are lacking the required number of convocation requirements, nor is it for students who have not completed UCCA 102 with a C- or better. Graduate and professional schools may have additional requirements. Deadline for submitting this completed form in time to have your name included in the commencement program is the same as the deadline for withdrawing from a course without academic penalty as stated on the official academic calendar.


 

Change of Incomplete Grade or Final Grade (Faculty/Staff only)
For faculty who wish to change a grade from INC or Z to a letter grade or from one letter grade to another.  Please note the following:

  • The form requires the signature of the instructor, plus the dean of the instructor's college (required only when making a change of grade other than the removal of an INC or Z).
  • The entire form must be submitted to the Student Records Office, who will in turn distribute copies to the student, instructor, and advisor.
  • The form can only be picked up from or dropped off to the Student Records Office by a Samford faculty or staff member--NOT by the student nor a student worker.

 

Grade Change Form (Faculty/Staff only)
For faculty who wish to give an incomplete (INC) grade to a student for a course being graded in the current semester.  Please note the following:

  • Incomplete grades are valid if the student has done work that would earn a passing grade in the course but has failed to complete some portion of the required work because of an emergency, and the work can be completed without further class attendance.
  • The student must complete the work in time for the instructor to grade it and submit a Change of Incomplete Grade or Final Grade form by the last day of classes in the next full semester OR the Incomplete will be changed to an F.
  • The form requires the signature of the instructor and, if available, the student.
  • The entire form must be submitted to the Student Records Office, who will in turn distribute copies to the student, instructor, department head, and dean.
  • The form can only be picked up from or dropped off to the Student Records Office by a Samford faculty or staff member--NOT by the student nor a student worker.
